Applicant Signature

The applicant must sign with their old name. For minors, parents or guardians should sign along with an attested legal guardianship order where applicable.

Self-Attestation

All supporting certificates and documents must be self-attested.

Eligibility

The applicant should be a native of the state and must submit a declaration.

Reason for Change

There must be a valid and genuine reason for changing the name.

₹750/- (As per G.O. Ms. No. 250, dated 11.10.2023)

Payment Method

In Person: e-Challan only (No cash). Send the counterfoil along with the form.

By Post: Payments by e-Challan only. No DD / Cheque / Postal Orders accepted.

Working Hours

Forenoon: 10:00 a.m. to 1:00 p.m.

Afternoon: 2:00 p.m. to 3:00 p.m. (No holidays)

Form Filling Method

Use block letters (capital letters) and black or blue ink if filling the form manually.

No Overwriting

Do not overwrite, strike out, or use correction fluid on the form.

Name Consistency

Old name and new name must exactly match the affidavit and newspaper advertisement.

Document Attachment

Attach affidavit, newspaper ad copy, identity proof, and address proof with the form.

Photocopy Requirement

Submit clear photocopies of all documents along with the original application.

Final Verification

Double-check all details before submission to avoid rejection or delay.
